&nbsp; <h2>Archway Veterinary Practice</h2> <p><strong>This form is for requesting refills of medication that has already been prescribed for your pet; new conditions will require an examination at the practice.</strong></p> <p>If your pet is due more of their usual medication,&nbsp;<strong>please order a minimum of 3 working days before collection</strong>, to allow time for medications to be ordered, dispensed and checked. If you have questions regarding your pet’s medications, please call us.</p> <p>Please be aware that by law:</p> <ul> <li>A veterinary surgeon is only able to prescribe veterinary medicinal products following a clinical assessment of an animal under their care.</li> <li>Pets on long-term medications will require a physical assessment with a veterinary surgeon every 12 months as a minimum.</li> <li>Some medications require a physical assessment with a vet every time they are prescribed, such as antibiotics and controlled drugs.</li> </ul> <p>The Small Animal written prescription fee is&nbsp;<strong>£27 inc VAT</strong>.&nbsp;You may obtain veterinary medicinal products from us OR ask for a written prescription and obtain these medicines from another veterinary surgeon or pharmacy.
